Welcome To Our Practice!
The initial consultation may take up to 2 hours depending on your history and the amount of testing required (skin tests, breathing tests, rhinoscopy, etc.). To allow us to provide a thorough evaluation during your first visit, please discontinue any allergy oral medications, nasal sprays or eye drops containing antihistamines for 4 days prior to your appointment date. Please note that many "cold & sinus" medications and sleep aids may also contain antihistamines. Please refer to our list of antihistamines to avoid or call us if you are unsure which medications to stop before your visit. If you have symptoms that are severe enough that you cannot discontinue antihistamine medications for the required number of days before your appointment, you can still be seen for a consultation but testing will have to be rescheduled to a later date.
What to Bring to Your Appointment:
When you present to the office for your appointment, please bring the attached form with you completely filled out as well as your insurance card(s) and a photo ID, usually a driver license. We will need a photo ID from a parent or guardian if the patient is a minor.
If your insurance plan requires you to obtain a referral from your primary care physician, please make certain that his/her office has generated this for you. For most insurance companies, we are able to retrieve the referrals on-line through Navinet. If you need a referral and one is not in the system for you or you have not brought a hard copy of it with you, your appointment will need to be rescheduled.

We would appreciate your help and courtesy if you are unable to keep an appointment. Please notify our office at least twenty-four (24) hours prior to the appointment time. We reserve the right to charge a missed appointment fee of $25.00 if 24-hours notice is not given.
Please refrain from wearing heavy fragrances to the office!
